Needs List for Loan Completion

Items needed

1. Copies of pay stubs past 30 days (2 most recent)

2. Copies of bank statements past 2 months (2 most recent)

3. Copies of recent savings, IRA, 401k, brokerage accounts or other reserves

4. Copies of employer issued w-2 forms past 2 years

5. Identity documentation - copy of drivers license, proof of valid social security number

6. Please forward information regarding homeowners insurance selection. Please supply insurance agent name, phone

7. Appraisal fee - $350 check made payable to "appraiser" - this can also be paid via credit card and the appraiser will call to get your information before the appraisal is conducted.

If applicable:

A. Self employed/ commissioned or other variable income supply copies of past 2 years tax returns

B. Child support or alimony income - provide copy of support order or divorce decree

C. Bankruptcy in past 10 years provide petition/ filing, discharge and written explanation

D. Social security or pension provide award letter and documentation of receipt

E. VA loans require military copy of the military discharge form DD214 and/or original certificate of eligibility

F. Exlplanation letter if late payments, collections or other derogatory items in the past 24 months
